At Sacred Ground Psychotherapy, we believe that transparency is a vital part of a trusting relationship. Part of our commitment to your care involves ensuring you have clear, easy access to your health information and a voice in your treatment process.

In accordance with Texas House Bill 4224, we want to ensure you know how to access your records or share feedback regarding your experience.

  • Your story belongs to you. If you would like to request a copy of your health records, please follow these steps:

    • How to Request: You can reach out to our administrator directly at admin@sgpsych.com or send a secure message through the Client Portal.

    • Delivery: To protect your privacy and maintain HIPAA compliance, we will deliver your records electronically through your secure client portal.

    • Important Note for Couples & Families: Because we value the privacy of every individual in the system, requests for records involving more than one person (such as marriage counseling) require the written consent of all parties involved.

    • Administrative Costs: To cover the time and resources required to compile and securely transmit your files, a standard administrative fee of $35 is typically requested for full record sets. We provide these records within 15 business days of your written request.

  • We strive to provide a space of healing, respect, and professional excellence. If your experience has not met these standards, or if you feel a "disconnection" in our process, we sincerely want to hear from you.

    • How to Reach Us: We invite you to share your concerns directly with your clinician or our administrative team at admin@sgpsych.com. If you would like to speak with leadership, one of our Clinical Directors is available to listen and respond personally. We view these moments as opportunities for growth and repair, and we are committed to finding a path forward together.

  • If we are unable to resolve your concerns together, or if you prefer to contact the state regulatory bodies directly, you have the right to do so:
